Meet your personal care team
Every Galileo member gets their own personal care team: a health advocate and lead provider. They’ll support you throughout your Galileo experience, along with our team of experts in specialty care, mental health, and social work.
Have your first visit
Your care team can come to your home, see you virtually via Zoom, or provide care in our fully-equipped mobile unit. They’ll ask you about your medical history and health goals, plus take vital signs and review medications.
Start your care plan
Your provider will go over your care plan with you, and schedule any follow-ups needed. We’ll stay in touch to make sure your plan is working for you.

Whenever you have a question or concern (even on nights and weekends), give us a call. We can take care of you over the phone, or make other referrals as needed.
